Chinese Visa Application Checklist for Tourism, Visiting Relatives/Friends & Business Trip (April 2008)

We suggest that you fill in and print the following checklist before you come to apply for visas. Or you can also fill it in at the counter.

I. I want to apply for "L" Visa (Tourism), and I provide the following documents,

* One completed application form, and I have filled in every column of this form and sign.

* Passport, and I know that the Consulate requires there should be at least 2 blank visa pages and enough validity time left (6-month validity for single-entry visa and 9-month validity for double-entry visa).

* One recent colour passport photo.

* Return tickets, copy of tickets or itineraries.

* Proof of hotel reservations in all the places I'm going to visit.

I understand that all the documents (except for the passports) will not be returned once the application is lodged, no matter the visa is granted or not. Signature by applicant: _______________

Or

I understand and will pass the message to the applicant, that all the documents (except for the passports) will not be returned once the application is lodged, no matter the visa is granted or not.

II. I want to apply for "L" Visa (Visiting Relatives), and I provide the following documents,

* One completed application form, and I have filled in every column of this form and sign.

* Passport, and I know that the Consulate requires there should be at least 2 blank visa pages and enough validity time left (6-month validity for single-entry visa and 9-month validity for double-entry visa).

* One recent colour passport photo.

* Return tickets, copy of tickets or itineraries.

* I was born in China.

* I was not born in China, and I provide Proof of Relations (e.g. Certificate of Marriage, Birth etc).

I understand that all the documents (except for the passports) will not be returned once the application is lodged, no matter the visa is granted or not. Signature by applicant: __________

III. I want to apply for "L" Visa (Visiting Friends), and I provide the following documents,

* One completed application form, and I have filled in every column of this form and sign.

* Passport, and I know that the Consulate requires there should be at least 2 blank visa pages and enough validity time left (6-month validity for single-entry visa and 9-month validity for double-entry visa).

* One recent colour passport photo.

* Return tickets, copy of tickets or itineraries.

* Invitation letter sent by the friend in China. The letter indicates the name, nationality, address, telephone number of the friend and the duration of the visit.

* Photocopy of the friend's passport or ID card, if he/she is a Chinese citizen.

* Photocopies of the friend's passport and visa, if he/she is not a Chinese citizen.

* The friend will accommodate me.

* Proof of hotel reservations since the friend will not accommodate me.

IV. I want to apply for "F" Visa (Business Trip), and I provide the following documents,

* One completed application form, and I have filled in every column of this form and sign.

* Passport, and I know that the Consulate requires there should be at least 2 blank visa pages and enough validity time left (6-month validity for single-entry visa and 9-month validity for double-entry visa).

* One recent colour passport photo.

* Return tickets, copy of tickets or itineraries.

* Proof of hotel reservations in all the places I'm going to visit.

* Official Visa Notification letter from the competent office of the Chinese government. (The inviter in China should get it and forward it to the applicant to support the visa application)
